On July 30, 2023, the CMRC coalition has submitted the following proposal to apply to Mayo Clinic's Social Innovation Proof-of-Concept Grant
Immaterial whether the concept ends up being accepted by Mayo Clinic's Social Innovation Grant, CMRC will continue to advocate co-design of systems, processes and digitial enablment to drive improvements in social determiinants of health in our community.

" ... your proposal “Community Health Collaboration Portal” has been selected to move on to the next round of evaluation for the Mayo Clinic Department of Medicine Social Innovation Grant.
For this next round, we invite you to pitch your proposal to our panel. In 20 minutes, an opportunity will be provided for a brief pitch, followed by an extensive question and answer session with our panel. Please see the attached document for more details on what to include for the pitch session.

We sincerely appreciate your interest in the Mayo Clinic Social Innovation RFA and the dedicated effort you invested in crafting your proposal and delivering your pitch. Your commitment to making a meaningful impact in our community is truly commendable, and it was evident in the strength of the proposal you presented.
Following a thorough discussion with the panel following the pitch presentations, we regret to inform you that, at this time, your proposal has not been selected for funding. The overarching feedback we received from the panel suggests that, strengthening the clear, unified vision for the proposed solution in this proposal could strengthen future applications. We encourage you to explore applying for the Commitment Against Racism Grant. The goal of this grant is to provide funding to address racism and benefit racial and ethnic minority groups in the county. Applications for this grant opened on Oct. 26 and will close on Nov. 10. You can also learn more about this and our other grant programs on the Community Relations website.
Please understand that this decision was not made lightly, as it was a very tight competition between the top proposals. We recognize the importance of the work you and your organization are undertaking, and we deeply respect your dedication.
